VITAL STATISTICS: DEATH CERTIFICATE INFORMATION

TO OBTAIN A HOCKING COUNTY DEATH CERTIFICATE BY MAIL:

(Death Certificates can only be ordered from the county of death)

• Complete a Certificate Application
• Mail to Hocking County Health Department.
• Include a self-addressed stamped envelope
• Forms of payment accepted:
  Credit card (w/ 3.5% transaction fee), check or money order payable to HCHD
• Orders are returned in the next business day’s mail
