TO OBTAIN A BIRTH CERTIFICATE STATE OF OHIO BIRTHS:

• Complete a Certificate Application
• Mail to Hocking County Health Department.
• Include a self-addressed stamped envelope
• A check or money order for $25.00 payable to HCHD
• Orders are returned in the next business day’s mail
