TO OBTAIN A DEATH CERTIFICATE OF A HOCKING COUNTY DEATH: 

Complete an application for the certificate.  Requests can be made in person at the Hocking County Health Department or can be mailed to us.  To obtain a certificate by mail, you must send an application or  letter of request along with a SELF ADDRESSED STAMPED BUSINESS ENVELOPE and a check or money order made payable to the Hocking County Health Department for $22.00. Certificates requested by mail have a next day turn around.

TO OBTAIN A DEATH CERTIFICATE OF A NON HOCKING COUNTY DEATH:

If you desire a death certificate not registered with the Hocking County Health Department but the death took place in Ohio, you may submit a request to the Ohio Department of Health. The address is:
