The Hocking County Health Department’s Vital Statistics office issues Death Certificates for deaths occurring in Hocking County from 1909 to present.

TO OBTAIN A DEATH CERTIFICATE OF A HOCKING COUNTY DEATH: Complete an application for the certificate. Requests can be made in person at the Hocking County Health Department or can be mailed to us. To obtain a certificate by mail, you must send an application or letter of request along with a SELF ADDRESSED STAMPED BUSINESS ENVELOPE and a check or money order made payable to the Hocking County Health Department for $22.00. If you desire a death certificate not registered with the Hocking County Health Department but the death took place in Ohio, you may submit a request to the Ohio Department of Health. The address is: Ohio Department of HealthRevenue Room246 North High Street, 1st Floor P.O. Box 15098Columbus, OH 43215-0098 Enclose a check or money order made payable to “Treasurer, State of Ohio” for the amount of $21.50. |
